What is the Ohio 50-hour affidavit?
The Ohio BMV 5791 Fifty-Hour Affidavit is required for all Ohio teen drivers under 21 applying for a probationary license. It confirms the applicant has completed the required supervised driving hours — 40 hours of daytime driving and 10 hours of nighttime driving.
In 2025 the BMV updated the form to require a detailed driving log documenting each practice session. The parent, guardian, or custodian signs the completed affidavit, and it must be notarized before the driving test appointment.
Notarization confirms the signer's identity and signature. It does not verify the accuracy of the hours logged.
Who needs the 50-hour affidavit notarized?
Any Ohio teen driver under 21 applying for a probationary license must submit a completed and notarized BMV 5791. The parent, guardian, or custodian who supervised the driving hours signs the affidavit — not the teen driver.
The teen must hold a TIPIC for at least six months before the driving test. Bring the completed, notarized affidavit to your driving test appointment.
