What is the difference between the 50-hour and 24-hour driving affidavit?

Ohio BMV Form 5791 (50-hour) is for teen drivers who complete supervised practice without a formal driving school program. Ohio BMV Form 5789 (24-hour) is for teens who complete a state-approved driver training program with classroom instruction and 8 hours with a licensed instructor. Ask your driving school whether their program qualifies for the 24-hour path.

Can I get a medical directive or living will notarized online?

Medical directives may be eligible for online notarization when your healthcare provider or attorney accepts RON. Living wills are more commonly required in wet ink by Ohio healthcare institutions. We confirm requirements with you before booking. For hospital and care facility visits, we coordinate with facility staff directly.

Can an I-9 be notarized online?

No. Federal law requires the employee to appear in person before an authorized representative for I-9 employment eligibility verification. Remote online notarization is not permitted for I-9 forms under current Department of Homeland Security guidelines. Mobile in-person appointments are available.

How do I know if my document needs witnesses in addition to notarization?

Witness requirements depend on the document type and your receiving institution. Power of attorney and some medical directives frequently require witnesses.
