Ohio requires teen drivers to complete supervised driving hours before getting a license. Whether your teen takes the 50-hour path or the 24-hour path through a state-approved driving school, the final step is the same: a notarized affidavit. Here's what you need to know.
The two Ohio teen driver license paths
50-Hour Path (BMV Form 5791)
Teens who do not complete a state-approved driver training school must log 50 hours of supervised driving practice — 40 hours daytime and 10 hours at night — with a parent or licensed adult. At the end, the supervising adult signs BMV Form 5791 before a commissioned Ohio notary. This is the default path for most teens and does not require enrollment in a formal driving school.

New 2025Teens who complete a state-approved driver training program (classroom instruction plus 8 hours with a licensed instructor) qualify for the 24-hour supervised driving path. They still need to log 24 hours of practice driving with a parent or licensed adult, documented on BMV Form 5789. After driving school — the affidavit step
Whether your teen completes the 50-hour or 24-hour path, the final step before submitting to the Ohio BMV is the notarized affidavit. The supervising parent or guardian signs the form before a commissioned Ohio notary, who verifies identity and witnesses the signature under oath. Do I need a driving school for my teen to get an Ohio license?
No. A driving school is not required in Ohio, but completing a state-approved program qualifies your teen for the faster 24-hour supervised driving path instead of the standard 50-hour path. Either way, a notarized affidavit is required before submitting to the Ohio BMV.
How do I know if a driving school qualifies my teen for the 24-hour path?
Ohio requires completion of a state-approved driver training program — 24 hours of classroom instruction plus 8 hours with a licensed instructor — to qualify for the 24-hour supervised driving path. Ask your driving school directly whether their program is state-approved and whether it qualifies for BMV Form 5789.
Where do I get the affidavit form?
BMV Form 5791 (50-hour) and BMV Form 5789 (24-hour) are available from the Ohio BMV website. Many driving schools also provide the form at completion of the program. Download links are on each affidavit page on this site.
Can I notarize the affidavit online even if the driving school was in person?
Yes. The notarization of the affidavit is separate from the driving program. Ohio authorizes Remote Online Notarization for both BMV Form 5791 and 5789. You complete a 15-minute video session, sign electronically, and receive the notarized form the same day.
