Teens who must log 50 hours of supervised driving practice — including at least 10 hours at night — typically use BMV Form 5791. The parent, guardian, or custodian who supervised the driving hours signs before a commissioned Ohio notary. Completing a state-approved driving school may affect your teen's training requirements; confirm with the BMV and your school which affidavit applies.

BMV Form 5789 (Twenty-Four Hour Affidavit) is for the abbreviated adult driver-training path — applicants age 21 or older who failed a first driving-test attempt and chose 24 hours of driving with a licensed driver. The applicant signs under oath. It is not the teen driving-school affidavit.

After driving school — the affidavit step
For most Ohio teen drivers, the supervising parent, guardian, or custodian signs BMV Form 5791 (Fifty-Hour Affidavit) before a commissioned Ohio notary. Do I need a driving school for my teen to get an Ohio license?
No. A driving school is not required in Ohio, but completing a state-approved program may affect your teen's driver-training requirements. Most teens document supervised driving on BMV Form 5791 (Fifty-Hour Affidavit), signed by the supervising adult before a notary.
Does completing a driving school change which affidavit my teen needs?
