What is the Ohio 24-hour affidavit (BMV Form 5789)?
The 24-hour affidavit (BMV Form 5789) is required by the Ohio Bureau of Motor Vehicles for teen drivers who complete the 24-hour supervised driving path. This path is available to teens who complete an approved driver training school — 24 hours of classroom or online instruction plus 8 hours with a licensed instructor. Do I need the 24-hour affidavit or the 50-hour affidavit?
It depends on which path your teen is taking. If they completed an approved driver training school (24 hours of instruction + 8 hours with an instructor), they use the 24-hour path and need BMV Form 5789. If they did not complete a driver training school, they use the 50-hour path and need BMV Form 5791.
