View all rates →What is the Ohio 50-hour affidavit?
The Ohio BMV 5791 Fifty-Hour Affidavit is required for all Ohio teen drivers under 21 applying for a probationary license. It confirms the applicant has completed the required supervised driving hours — 40 hours of daytime driving and 10 hours of nighttime driving.
In 2025 the BMV updated the form to require a detailed driving log documenting each practice session. The parent, guardian, or custodian signs the completed affidavit, and it must be notarized before the driving test appointment.
Notarization confirms the signer's identity and signature. It does not verify the accuracy of the hours logged.
Who needs the 50-hour affidavit notarized?
Any Ohio teen driver under 21 applying for a probationary license must submit a completed and notarized BMV 5791. The parent, guardian, or custodian who supervised the driving hours signs the affidavit — not the teen driver.
The teen must hold a TIPIC for at least six months before the driving test. Bring the completed, notarized affidavit to your driving test appointment.

Where do I get the official Ohio 50-hour affidavit form?

What changed on the 2025 Ohio 50-hour affidavit?

What if online notarization is not accepted at my BMV?
Request a mobile notary visit for in-person notarization in Greater Cincinnati, or contact your local BMV location to confirm their requirements before booking.
What is the 24-hour affidavit and do I need it too?
Ohio BMV Form 5789 is a separate affidavit required for teen drivers who complete the 24-hour supervised driving path (available when the driver completes an approved driver training school) instead of the standard 50-hour requirement. If your teen is using the 24-hour path, you will need BMV 5789 notarized instead of — or in addition to — BMV 5791. We notarize both. The process and fee are the same.
Remote online notarization is available for eligible documents. Document eligibility varies. Some receiving agencies may have additional requirements beyond notarization. We review document details if needed before sending your secure remote session link.
